Position Summary General Summary/Responsibilities:
The Outpatient Social Worker provides psychosocial assessment, counseling, care coordination, crisis intervention, and resource connection services to patients and families receiving care in the outpatient setting. This role supports patients’ emotional and practical needs to improve health outcomes and quality of life.

Education
Minimum Level of Education Required: Master's degree Additional requirements:

▪ Preferred educational qualifications: Bachelor’s or Master’s Degree in Social Work
▪ Position specific testing requirement: Licensure/Certification/Registration
▪ Licensure LSW, LMSW, LCSW Experience
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: N/A
Prior specific functional responsibilities:
Previous outpatient, hospital, community health, or mental health experience preferred

Essential Duties & Functions:
• Conduct initial and ongoing psychosocial assessments for patients referred by providers or care teams.
• Provide education, consultation and acts as a liaison between the provider, patient and family, community agencies and the multi-disciplinary team.
• Develop and implement individualized care plans, including mental health, social service, and support needs.
• Provides assessment and referral in cases of abuse, neglect, or exploitation and follows Ohio mandated reporting laws.
• Provide brief therapy, supportive counseling, and crisis intervention as appropriate.
• Identify and connect patients to community resources, including housing, financial assistance, transportation, and mental health services.
• Assist patients in navigating insurance barriers and advocate for necessary services.
• Respond promptly to urgent/emergency patient situations.
• Maintain thorough, timely, and compliant documentation within the electronic medical record system.
• Facilitate patient education sessions on wellness, mental health, coping strategies, and available community resources.
• Monitor patients’ progress toward identified goals and adjust interventions as needed.
• Maintain current knowledge of community resources, eligibility criteria, and referral processes.
• Follow all applicable ethical guidelines, privacy laws (HIPAA), and organizational policies.
