General Summary/Responsibilities:

The Chief Nursing Officer, Sr. Director, Clinical Operations is responsible for the leadership and direction of all Fidelity clinical services / operations.  Departments of responsibility include but not limited to: (1) Clinical Home Health Field Operations, (2) Clinical Rehabilitation Services, (3) Clinical Education and Quality, (4) Clinical Infusion Services, (5) Patient Care Coordinators and (6) Scheduling department and (7) Respiratory Therapists.

The Chief Nursing Officer, Sr. Director, Clinical Operations manages all inpatient care services, case management, disease management, quality of care management (outcomes, metrics, programs, projects, process improvement, etc.) and the overall administration and regulatory functions of the company (CMS certification, TJC Accreditation, safety, infection control, Board Quality and compliance).   In addition, the Chief Nursing Officer, Sr. Director, Clinical Operations is responsible for the administrative management of the Clinical Coordination Center – referral/intake and scheduling.

In collaboration with other members of Fidelity Leadership team, assumes responsibility for achieving desired clinical outcomes, financial and employee engagement metrics in assigned areas of responsibility.

  • Quality Outcomes include but not limited to; trending or achieving 5-star CMS service levels, favorable value-based purchasing performance score results and favorable patient experience outcomes.
  • Financial Outcomes include but not limited to; meeting or exceeding home health budged volume metrics (referrals per month, LUPA per referral, etc.), revenue per admission and cost per effective processes.
  • Employee Satisfaction include by not limited to; achieving employee engagement strategic goals, meeting or exceeding budgeted home health hiring and retention budgeted goals. 

Provides administrative oversight to the programs to develop and maintain relationships with Premier hospital facilities and implement operational processes to improve productivity and customer service.  Facilitates post-acute programs to support Premier Health hospitals.  This position is also responsible for the development, implementation and management of multiple divisions departments, programs, and interdisciplinary teams, to provide care coordination, hospital inpatient and emergency department discharge coordination to home health and durable medical equipment services, value-based initiatives, Readmission Reduction initiatives, Chronic Care Management and Transitional Care Management of the Premier Physician Network patient populations, and selected payor populations within the community based and ambulatory areas of Premier Health.    

Education

Minimum Level of Education Required:  Master's degree

  • Area of study or major: Nursing and or Business Administration 

Licensure/Certification/Registration

  • Registered Nurse

Experience

 

Minimum Level of Experience Required: 10+   years of job related experience

Prior job title or occupational experience: Home Care clinical management experience of at least  7+years.

Preferred experience: Prior Home Health experience

Knowledge/Skills

  • Excellent oral and written communication skills to allow accurate exchange of information
  • Excellent organizational skills
  • Excellent customer service skills (internal and external).
