Education

Minimum Level of Education Required: Diploma Program                                            

Additional requirements:

    • Type of degree: Diploma, Associate’s, Bachelor’s

Licensure/Certification/Registration

 

  • Current Licensure (LPN, RN, LSW) in the State of Ohio

 

Experience

               Minimum Level of Experience Required: 1-3 years of job-related experience

Prior job title or occupational experience: Experience with providing clinical case management services in a community/public health setting preferred. Experience in Breast Health, Oncology, or Women's Health a plus. Experience in a clinical educator role and a site operation lead role preferred.

  • Ability to effectively provide case management services to socially and medically vulnerable clients
  • Ability to work collaboratively in a team and manage multiple priorities, utilize effective time management, and exercise sound clinical judgment
  • Demonstrated ability to work well with people of various ages, backgrounds, ethnicities, and life experiences
  • Exceptional organizational and interpersonal skills, with attention to detail require; strong oral/written communication skills is a must
  • Ability to analyze and synthesize current clinical standards and guidelines and communicate clinical education updates to staff
  • Computer skills necessary

Organizational Standards

 

 

  • BCCP is a competitive federal grant from the CDC and flows through the Ohio Department of Health.
  • Continuation of this position is dependent on continuation of grant funding
  • Determines participant eligibility according to the ODH guidelines set for this project.  Responsible for obtaining appropriate information from participant to complete ODH eligibility forms
  • Conducts health history interview and identifies appropriate screening examination requirements
  • Facilitates obtaining screening and diagnostic test results from provides
  • Ensures proper follow-up and referral of abnormal screening examinations
  • Completes final diagnostic and treatment reports in Med-IT
  • Assists clients in enrolling in BCCP Medicaid or refer to alternative financial resources
  • Assists clients in obtaining community resources, which may include transportation, interpretation, and support services
  • Participates in recall activities for surveillance of abnormal screening examinations
  • Identifies patients in a psychosocial or medical crisis and refers to appropriate health professional in a timely way
  • Communicates appropriately with other professionals to discuss relevant aspects of BCCP Services.
  • Completes all documentation accurately, completely, and in a timely manner in Med-IT
  • Performs other related duties required to support the SW BCCP

 

Essential Duties & Functions

 

Essential Duties & Functions:

 

 

The incumbent will be responsible for high-quality clinical case management for the SW Breast and Cervical Cancer Project. 

 

Other Duties & Functions:

  • Provides training to the community screeners on proper use of all medical field equipment and standard fingerstick procedures and protocols and ensures all screeners maintain clinical competence on all screenings and machines
  • Provides continuing education modules for screener staff; and addresses/resolves any learning gaps or needs among screening staff
  • Ensures all screening values and recommendations are current and adjusted as national standards are adjusted
  • Designs and makes ongoing updates to education materials for learning modules
  • Liaisons with PCH’s Logistics Coordinator and Program Specialist, and supply distributors to ensure that screening equipment is tested and is in good/accurate working condition, in compliance with stated manufacturer guidelines
  • Collaborates with Program Coordinator in ensuring all educational needs are met for each individual screener
  • Serves as the team lead and works in the field, including the Mobile Clinic, ensuring that all events operate smoothly
  • Troubleshoots and liaisons between the field staff and the office staff
