Qualifications
Education
Minimum Level of Education Required: Associate degree
Additional requirements:
Type of degree: Social Work, Counseling or related field
Area of study or major: Human and/or social services
Licensure/Certification/Registration
CDCA or Licensed Chemical Dependency Counselor II (LCDC II) or higher or a Bachelor Level LSW or LPC.
Experience
Minimum Level of Experience Required: No prior job-related work experience
Prior job title or occupational experience: 3-6 months experience in the mental health and/or drug and alcohol field
Prior specific functional responsibilities: N/A
Preferred experience: 1-2 years’ experience in the mental health and/or drug and alcohol field

Knowledge/Skills
Working knowledge of mental health, drug and alcohol, and other community agencies and resources.
Excellent interpersonal skills, critical thinking and ability to communicate in writing.
Excellent organizational skills.
Must be physically and mentally able to competently deal with verbally and physically aggressive patients
Computer keyboarding or typing skills recommended
Knowledge of the behavioral health field and best practices
Essential Duties & Functions:
Provide care coordination services in the form of face-to-face, office and field-based, telephone calls, telehealth visits, report writing and care coordination conference meetings.
Act as the single point of contact for clients and appropriately coordinate with the care coordination team members as well as the broader care team to support the treatment and recovery plan. Appropriately engage peer specialists, nursing, and any other specialties, based on clinical need.
Collaborate with referral source, internal team members and external service providers to ensure appropriate client care and continuity of care.
Assist individuals with identifying barriers to full participation in community resources and using creative problem-solving to develop strategies to overcome those barriers.
Actively identify and support linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.) that support the individual’s goals and interests. Collaborate with internal team members from other disciplines when in doubt of appropriate linkage to services.
Provide vision-driven hope and encouragement for opportunities at varying levels of involvement in community-based activities (e.g., work, school, relationships, physical activity, self-directed hobbies, etc.).
Provide recovery education to individuals for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance
In conjunction with assigned therapist, assist individuals in developing their own plan for advancing their recovery.
Participate in required meetings, trainings, quality improvement initiatives, and case reviews as needed.
Perform urine drug screens as requested.
Complete all documentation in the required time frames
