JOB QUALIFICATIONS & EXPERIENCE REQUIRED:
Two year degree in a relevant social science field or equivalent education and experience. Preferred degree in social work.
CDCA, Licensed Chemical Dependency Counselor II (LCDC II) or higher or a Bachelor Level LSW or PC.
Minimum of two (2) years experience in the mental health and/or drug and alcohol field.
Working knowledge of Montgomery County mental health, drug and alcohol, and other community agencies and resources.
Working knowledge of The Diagnostic and Statistical Manual of Mental Disorders (DSM- 5), as related to coordination of community support services.
JOB DUTIES:
Provide care coordination services in the form of face to face, office and field-based, telephone calls, report writing and care coordination conference meetings. Manages and guides other care coordinators.
Collaborate with referral source, internal team members and external service providers to ensure appropriate client care and continuity of care.
Complete all direct service paperwork within agency timelines or earlier if referral source or other healthcare provider necessitates.
Act as the single point of contact for clients and appropriately coordinate with the care coordination team members as well as the broader care team to support the treatment and recovery plan. Appropriately engage peer specialists, nursing, and any other specialties, based on clinical need.
Attend OneFifteen trainings and participate in quality improvement procedures
Demonstrate strong customer satisfaction skills in the provision of clinical care services.
Demonstrate knowledge of SBHI medical records and EMR and other documentation standards. Apply competencies in both the provision and documentation of medical records.
Demonstrate compliance with departmental standards for administration and reporting outcome measures and documentation of utilization management of clinical care.
Demonstrate knowledge of regulatory standards and ensure that clinical records are maintained in compliance with these standards at all times.
Demonstrate knowledge of IT and Quality Improvement policies and procedures. Apply them to the documentation of clinical care according to medical records and other departmental standards.
Assess current competencies. Design and implement professional development plan to improve competencies in targeted goal areas to support departmental strategies.
Participate in weekly multidisciplinary care team meetings Meet regularly with clinical supervisor and as needed to review clinical care of clients according to licensing and agency standards.
