CASE MANAGER-SBHI
Case Manager functions as a member of a team to assist in the planning, development, and delivery of outcome-effective treatment services un various types of settings. This position maintains an ongoing working knowledge of agencies and community/emergency resources for clients and uses that knowledge to coordinate services and advocate for identified clients. Case Managers acts as a resource consultant to other team members and collects resource materials regarding other agencies and services in the community. Services may be delivered by phone, fact-to-face at the office, in patients' home, at outside agencies, within school setting and/or within the community.
