CLIN PHARM SPEC-CARDIOVASCULAR
Under the supervision of the System Director, Pharmacy Clinical Services and according to standard procedures, provides clinical pharmaceutical care; coordinates pharmaceutical care with cardiovascular health care providers, practitioners, and educators to ensure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective medication therapy; optimize medication use through drug order review, therapeutic drug monitoring, participation in high-risk emergencies including resuscitation; supervises support personnel in all work areas assigned; precepts pharmacy residents and pharmacy students; participates in clinical research protocols and drug therapy research activities as applicable. The clinical pharmacy specialist provides for staff development activities and the coordination of staff operational activities within cardiovascular services; serves as educational and informational resource to physicians, medical residents, advanced practice providers, pharmacy staff, nursing staff, patients, and other allied health professionals; assists with planning, organizing, and implementation of clinical pharmacy programs across Premier Health; identifies processes and tools to guide the pharmacy staff in the utilization of the most appropriate and cost-effective medication therapy including drug preparation and procurement, ensuring high quality, safety, and positive patient outcomes.

· Completion of a post-graduate year 2 residency (PGY2) or fellowship required -or-
· Completion of a post-graduate year 1 residency (PGY1) plus minimum of 2 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring -or-
· Minimum of 3 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring
Knowledge/Skills
· Fluent in the use of drug information resources
· Ability to evaluate primary medical literature and develop evidence-based recommendations for treatment regimens
· Ability to perform Medication Use Evaluations (MUE) and submit recommendations for change within the health system
· Ability to provide pharmacokinetic and weight-based dosing
· Possess good interpersonal skills, excellent verbal and written communication skills, and
problem- solving skills
· Ability to read and write printed material in English
· Ability to effectively perform patient interviews and medication counseling
· Ability to work well with other health care workers and perform well within a team
· Proficient with bedside aseptic technique
· Must be customer-service oriented
