Facility: Miami Valley Hospital
Under general supervisor, preps, scans, and validates all types of paper documents into optical scanning system to obtain chart completion. Scans & validates all loose reports that are sent to the department. Responsible for analyzing medical records for deficiencies in chart documentation. Responsible for release of information for continuity of care requests. Assures that guidelines of HIPAA are followed to reduce the hospital’s potential liability in releasing records or protected health information. Responsible for monitoring incomplete charts and follow up with physicians. Responsible for following guidelines of JC and Conditions of Participations and all federal and state. Responsible to perform other duties as assigned and needed.

Minimum Level of Education Required: High School completion / GED

Experience
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Medical Office
Preferred experience: Health Information or related medical field, Knowledge of Word & Excel.
Other experience requirements: Must have completed a course in medical terminology or be enrolled in a class at the present time.
Knowledge/Skills
- Ability to read and write in order to review and maintain patient medical record charts, and perform basic mathematic calculations as well as ability to operate standard office equipment as a level normally acquired through completion of high school.
- Must be able to work independently in a medical office setting.
- Problem-solving abilities and basic analytical skills necessary.
- Must possess excellent organizational skills.
- Ability to type accurately at acceptable levels of speed.
- Strong verbal and written communication skills.
- Interpersonal skills required to effectively communicate, understand, interact, and respond to patients, physicians, and other personnel when obtaining and supplying routine factual medical record information via telephone or direct personal contacts on a daily basis.
- Basic analytical skills necessary in order to determine work priorities, assist in problem resolution and recommend methods of improving workflow.
