Qualifications

 

Education

 

Minimum Level of Education Required: Associate degree

                                               

Additional requirements:

§  Type of degree: Associates

§  Area of study or major: RN or LPN

§  Preferred educational qualifications: BSN or LPN

§  Position specific testing requirement: N/A

 

Licensure/Certification/Registration

 

§  Current Ohio RN or LPN License.

 

Experience

 

Minimum Level of Experience Required: 1 - 3  years of job-related experience

Prior job title or occupational experience: RN or LPN

Prior specific functional responsibilities: N/A

Preferred experience: One year experience in clinical/ medical setting. Home health experience preferred.

Organizational Standards

 

§  Customer Service Objectives

o   Complies with all Fidelity Health Care Customer Relations Standards (see attached addendum).

§  Referral Processing

o   Conducts daily rounds and reviews assigned patient records.

o   Completes clinical patient assessment and determines potential service needs in the home environment.

o   Communicates plan of care established by physician to appropriate hospital and agency staff.

o   Evaluates reimbursement options and educates patients regarding cost.

o   Provides education to patient and/or caregiver regarding home care services and expectations.

o   Coordinates care with agency (ies).

o   Identifies problems early and works to facilitate smooth transition of patients into the home care environment.

o   The incumbent demonstrates ability to promote Fidelity Health Care programs and services confidently and with accuracy.

o   Identifies new home care opportunities to meet needs of patients.

o   Assists in education of physicians, hospital staff, and managed care partners about services provided by Fidelity and other PHP outpatient services and facilities.

o   Weekend and holiday work may be required.

Essential Duties & Functions:

 

This position requires interaction with administrative staff, hospital staff, and medical staff.  Time and skills required to achieve objectives is as follows:

§  Processes home health, durable medical equipment and infusion referral volume received from discharge planning, physicians and hospital staff.

§  Provides appropriate feedback to physician, hospital staff, and agencies regarding patient progress and addresses concerns communicated.

§  Actively educates regarding clinical programs by initiating contact with physicians and staff.

§  Identifies program development opportunities, through work with the manager and specified hospital clinical program work groups.
