Job Title: Home Visit Coordinator - RN
Unit: HMGBF Expansion

The Home Visit Coordinator (HVC) functions in a multifaceted role that demonstrates professionalism.  The Home Visit Coordinator (HVC) is a reflective and administrative supervisor of the Nurse Home Visitors in the Help Me Grow Brighter Futures Program.  The HVC collaborate with each nurse home visitor on client care review, progress and planning; as well as, home visitor reflection, processing and development.  Supervising home visits will be done on a regular basis.  The HVC is responsible for maintaining high standards in clinical practice following the program model.  The HVC is responsible for assuring all staff follow policy, procedures, protocol and rules of the respective employer and program.  Has oversight of new staff orientation staff education and competencies and PI chart monitoring.  The HVC will support compliance with data entry. 

Essential Duties & Functions:

Meet with Nurse Home Visitors for supervision – both administrative and reflective. 

Create a positive work environment that promotes productivity.

Receptive to new ideas and methods and willing to adapt to change.

Oversee compliance to policies, procedures, protocols, and standards of Premier Health, Help Me Grow Brighter Futures, GDAHA and the home visiting program. 

Provide regular supervisor visits with staff in accordance to their program model (at least annually), and more often on new staff.

Oversee new staff orientation, staff education and competency.

Promote a seamless continuum of care between all aspects of care and community resources. 

Adhere to policies concerning confidentiality and consumer relations.

Collaborate with PI in chart monitoring and compliance requirements. 

Monitor staff productivity. 

Contribute to the efficient, orderly operations of the department

 

Education Required: Bachelor's degree                                             

Additional requirements:

Type of degree: Bachelor's degree in nursing 

Area of study or major: Nursing or related field

Preferred educational qualifications: Master's degree in nursing 

Position specific testing requirement: Must undergo a criminal background check with the Ohio bureau of criminal identification and investigation and a background check with the Ohio central registry on child abuse and neglect.   

 Licensure/Certification/Registration: Licensed Registered Nurse  

Experience Required: 3 - 5  years of job related experience

Prior job title or occupational experience: Home visiting.

Prior specific functional responsibilities: At least 1 year working with pregnant women, infants, and children.

Preferred experience: Supervisory experience.

Knowledge/Skills: 

·        Must possess and display effective verbal, written and interpersonal communication skills. 

·        Organized and self-directed. 

·        Valid driver’s license and insured safe automobile.

·        Non-smoker
