The Licensed Practical Nurse (LPN) functions as a member of the medical services team to assist in the provision of clinical care, intervention, educational services and care coordination to child, youth and adult patients. The LPN collects data relevant to the delivery and promotion of medical services.

Job duties:

Demonstrate adherence to the PHP/SBHI Patient Experience philosophy, guidelines and tools.

Demonstrate strong ethics, honesty, and adherence to PHP/SBHI Code of Conduct and PHP/SBHI Compliance Program.

In coordination with providers and other staff members ensure smooth and efficient patient flow.

Complete all direct service documentation within established agency and departmental guidelines, including use of CareLogic.

Review and sign patient Health Histories. Make follow-up recommendations to patients, appropriate staff, physician, and/or NP.  Enter allergies and medications as reported by the patient into CareLogic.

Provide billable case management (CPST) and other pharmacological management services in the form of face-to-face interventions and telephone calls.

Demonstrate clinical knowledge and skills relevant to clinical nursing services and intervention methods provided to patients.

Based on need, manage orders, including calling-in medications, educate patients regarding proper use of medications, side effects, and coordinate lab work according to departmental standards.

Seek consultations regarding medical needs of patients. Maintain on-going communication with medical providers for the purpose of coordinating and evaluating patient care.

Demonstrate knowledge of SBHI and departmental medical records and other paperwork standards and apply competency in both the provision and documentation of medical information in CareLogic.

Demonstrate knowledge of regulatory standards. Ensure that clinical records are maintained in compliance with these standards at all times.

Respond to patient inquiries and collaborate with physicians/NPs regarding patient information.

Attend required staff meetings and trainings.

Maintain Medication Logs for sample medications, including signing-in and tracking of samples. Submit the Medication Discrepancy Log on a monthly basis.

Destroy outdated sample medications per SBHI policy.

Complete medication pre-authorizations in a timely manner.

Demonstrate knowledge of quality improvement and information technology policies and procedures and apply them to the documentation of clinical care according to medical records and other department standards.

As a result of patient cancellations, back-fill physician/NP schedules when needed.

JOB QUALIFICATIONS & EXPERIENCE REQUIRED:

Graduate from an accredited Nursing Program.

Current licensure as a Licensed Practical Nurse in the State of Ohio.

Training and experience in the types of services provided by the team, with age appropriate competency, using the Diagnostic and Statistical Manual of Mental Disorders (DSM- 5 or subsequent revisions).

Demonstrated knowledge of psychiatric and medical drugs.

WORKING CONDITIONS AND EXPECTATIONS:

There is generally a pleasant physical environment; however, unpleasant conditions may include frequent interruptions, difficulty controlling office temperatures, heavy workload demands and constantly changing regulations and standards.

While performing the duties of this position, the employee is required to talk and/or hear, to sit, stand, and walk; use the hands to finger, handle, and/or feel and be able to reach with hands and arms.  Specific vision abilities required by this position include close, distance, and peripheral vision and the ability to focus visually.

Some flexibility in scheduling is expected to meet changing SBHI business needs.  This may require employees to work different days/hours or stay beyond their normal shift. 

Must be able to work cooperatively and effectively within a team as demonstrated by effective interpersonal, communication and organizational skills.

Demonstrated ability in basic computer literacy skills.

Influenza vaccination is required on an annual basis.

Physical harm may result from aggressive or physically ill clients.  Aggressive client training and adherence to universal precautions helps to minimize the risk of physical harm.

As applicable to job duties, valid Ohio driver’s license and/or dependable private vehicle is required.

As applicable to job duties, Crisis De-escalation training is required on an annual basis.

As applicable to job duties, maintain current certification in First Aid and CPR.
