MED SOCIAL WORKER-BSW
Provides direct and indirect clinical social work and transitions planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel.
Frequent contact with patient care services staff, physicians, and ancillaries to discuss patient care and diagnostic needs. Participate in care rounds and other interdisciplinary collaboration within the in-patient and outpatient programs. Frequent contact with community agencies, third party payers, outside review organizations and case managers
