MED SOCIAL WORKER-BSW
The medical social worker provides direct social work intervention, transition planning and counseling to patients and their families. The medical social worker assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning. The medical social worker assists patients and families in making appropriate linkages to community resources and provided consultation to those seeking input and guidance in efforts to resolve patient concerns. Transition planning and social work services are provided to inpatient and outpatient populations of the hospital.

Minimum Level of Education Required:
Bachelor's degree

Pursuant to signing an agreement to complete MSW degree within 3 years of hire.
