Organizational Standards
§
Provide psychosocial assessment and
ongoing intervention to assist in restoring patients to their optimum level of
social and health functioning. This
includes assisting patients and their families to understand, accept and
follow medical recommendations.
§
Provide counseling and intervention to
complex psychiatric patients and assists them in understanding, accepting, and
following treatment regimen.
§
Direct patients toward utilization of
family and community resources. This
may be accomplished by assessment and referral of the patient to resources or
acting as a patient advocate in working with other health and welfare
agencies.
§
Assist patients and their families
with personal and environmental difficulties which predispose them toward
illness or interfere with achieving maximum level of functioning. These range
from counseling members of the patient's family to assisting patients with
admission to a nursing home.
§
Consult with the physician and other
members of the health care team for the purpose of assisting them to
understand significant social, emotional and environmental factors related to
the patient's health problems.
§
Assist in the development and revision
of the physician's plan of treatment.
§
Prepare clinical/progress notes using
the tools and processes established by Fidelity Health care, provide summaries
and reevaluations if indicated, and submit documentation according to
established policy.
§
Meet on a regular basis with clinical
supervisor to review case workload.
§
Provide Notary Public services to
patient/family members in order to complete Advance Directives.
§
Attend interdisciplinary case
conferences to plan patient care.
§
Attend mandatory staff development
activities.
§
Attend scheduled staff meetings.
§
Participate in multidisciplinary
process improvement teams as appropriate.
§
Provides Medical Social Worker
representation and leadership for Fidelity Health Care in professional
settings as appropriate.
§
Provides consultation to Fidelity
clinicians on complex patient care situations.
