NETWORK TRANSITION MANAGER
Position Summary
General Summary/Responsibilities:
The Network Transition Manager oversees, coordinates, and leads all aspects of the provider pre-boarding, onboarding and offboarding lifecycle. He/she is responsible for developing initiatives and programs to ensure smooth, efficient, and compliant transitions for practices, physicians, and providers within Premier Health. This role involves developing and overseeing provider and practice preboarding, conducting workflow assessments and gap analysis, and driving process improvement by partnering with leaders across multiple departments. This individual will use data and analytics to monitor performance and implement best practices. The manager coordinates and aligns various transition activities, acts as a liaison to PPN executive leadership, and collaborates closely with leadership across Premier Health. Strong communication is critical in this role.
The Network Transition Manager develops, implements, and continuously improves physician transitions to enhance new hire experience and retention. The manager coordinates and leads all aspects of transition including credentialing, privileging, mal-practice, compliance documentation, and orientation schedules. The manager collaborates with multiple departments across the system to ensure timely completion of pre-boarding, onboarding, offboarding, and transition tasks while serving as a primary point of contact.

Essential Duties & Functions:
• Leads all aspects of physician, provider, and practice preboarding, onboarding, and offboarding.
• Ensures that the provider and staff experience is seamless and supportive.
• Leads and provides program management of the transition process (preboarding, onboarding, and offboarding), including but not limited to: credentialing, privileging, mal-practice, human resources, patient communication, marketing, digital health and technology, medical staff office, compliance, and orientation.
• Communicates timelines, expectations, and coordination between multiple departments.
• Demonstrates strong teambuilding and organization skills
• Exhibits exceptional communication skills to support physician, provider, and staff experience and integration.
• Partners closely and interact with multiple departments across Premier and PPN.
• Comfortable in public speaking
• Ability to work positively with cross-functional teams to achieve established corporate goals
• Coordinate logistics, resources, and stakeholder engagement to ensure new physicians, providers, and teams have the tools, information, and connections needed to succeed in their roles.
• Design and facilitate transitions meetings (virtual and in-person)
• Lead or co-lead high-impact initiatives that drive operational excellence across Premier Physician Network. This role plays a key role in assessing performance, improving key processes, and enhancing the overall efficiency across Premier Physician Network’s transitional areas.
• Work closely with cross-functional leaders to identify and execute improvement strategies that align with Premier Health goals.
• Partner with operational leaders to evaluate business processes, identify pain points, prioritize initiatives, and propose solutions that enhance quality, efficiency, and patient experience.
• Analyze team performance and metrics to inform data driven decision-making and evaluate impact of improvements. Translate data into meaningful insights and visualizations that guide business leaders toward action.
• Propose actionable recommendations and help facilitate the implementation of necessary changes.
Qualifications
Education
Minimum Level of Education Required: Bachelor's degree
Additional requirements:
Type of degree: Masters Preferred
Area of study or major: Business, Healthcare Operations, Nursing
Preferred educational qualifications: Masters degree in Business, Healthcare Operations, or Nursing is preferred
Experience
Minimum Level of Experience Required: 5 - 7 years of job related experience
Prior job title or occupational experience: Operations, Healthcare Operations, or Nursing
Prior specific functional responsibilities: Experience in operations and provider transitions
Preferred experience: Leadership, operational, and physician relationship experience in a large complex health system
Knowledge/Skills
• Understanding physician practice functions, processes, best practices, and key performance indicators.
• Strong communication skills, including the ability to partner and communicate with physicians.
