Occupational Therapist/ Home Care Case Manager
General Summary/Responsibilities:
The Occupational Therapist evaluates and then directly provides or supervises the provision of occupational therapy services to patients in their homes or residences. When functioning in the capacity of the Home Care Case Manager (HCCM), the Occupational Therapist is responsible for the provision and coordination of all components of a patient’s home care services. The Occupational Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

Education
Minimum Level of Education Required: Bachelor's degree
Additional requirements:
Type of degree: Bachelors - Graduate of an accredited Occupational Therapy Program
Area of study or major: Occupational Therapy
Licensure/Certification/Registration
Current licensure to practice Occupational Therapy in Ohio
CPR Certification
Experience
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care.
Preferred experience: One year clinical experience in acute care setting preferred
Knowledge/Skills
Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.
Must be able to creatively adapt multiple environments and situations for successful outcomes.
Must be self-directed and able to work with little supervision.
Computer skills preferred.
Home care experience preferred.
