ONCOLOGY NURSE NAVIGATOR-PPN
Summary
The Oncology Nurse Navigator is a registered nurse (RN) who provides comprehensive, patient‑centered care coordination, navigation, and direct clinical nursing services across the outpatient oncology and ambulatory care continuum. This role supports patients and families from diagnosis through treatment and follow‑up, ensuring care is timely, appropriate, safe, and coordinated while optimizing patient outcomes, experience, and quality measures.
The RN practices to full scope of licensure, serves as a clinical expert and patient advocate, partners with multidisciplinary teams, and supports quality improvement initiatives in alignment with organizational goals and regulatory standards.
The RN partners with and advocates for patients/families/significant others to provide physical, emotional and spiritual support which impacts patient outcomes. The RN is responsible for seeing that patients receive treatment services that are timely, appropriate, and comprehensive. This is accomplished through identification of patients’ needs and working collaboratively with a multi-disciplinary team across the in-patient and out-patient continuum of care. The RN supports patient/family education, monitors the recommended treatment plan, and sees that appropriate follow up occurs in the outpatient setting. The RN also monitors identified quality measures and works with the multi-disciplinary team to reach expected outcomes. The RN functions in a versatile role that demonstrates professionalism and promotes excellence and autonomy in the practice of Nursing. The RN utilizes the nursing process in conjunction with evidence-based principles to care for a diverse caseload of patients within the department's scope of service. The RN serves as guide, healer, collaborator, teacher, leader and sentry. The RN collaborates with and guides care delivered by the inter-professional team. The RN is accountable for compliance with the Ohio Nurse Practice Act, Nursing Code of Ethics, and applicable regulatory standards.
