General Summary/Responsibilities:

The Perinatal Outreach Nurse (PON) is responsible for providing comprehensive care prenatally ending at two years of life. The PON completes postpartum and newborn head to toe assessments, offers support, education, and service coordination for clients through in home visits. This position aims to promote family well-being, child development, and access to community resources to empower pregnant people and families with young children to thrive. Responsibilities include:

· Home Visiting: Conduct regular home visits to assess vitals, family needs, support parenting practices, provide education and referrals, monitor child development, and engage families in evidence-based curriculum.

· Assessments:

o Pregnant women: weight, blood pressure, edema, fetal movement, breasts, auscultation of heart and lungs for irregularities, and signs of preeclampsia or other concerns in pregnancy.

o Postpartum women: BUBBLE- breasts, uterus, bladder, bowels, lochia, episiotomy/c-section incision, vitals, infection, general appearance, emotional well-being, bonding and pain.

o Newborn assessment: weight, length, skin, eyes, auscultation of heart and lungs for irregularities, abdomen, genitals, nervous system and reflexes, and feeding (breast/bottle).

· Service Coordination: Develop, implement, and monitor individualized service plans based on family goals and needs and support and/or facilitate transition to needed services.

· Resource Referral & Coordination: Connect families with appropriate community resources such as prenatal care, lactation, postpartum care, pediatrics, dental, vision, hearing, developmental, housing, employment, education, legal, primary care, and mental health services.

· Documentation & Reporting: Maintain accurate and timely records of all interactions, assessments, referrals, and follow-ups in accordance with program guidelines.

· Advocacy & Support: Advocate on behalf of families to ensure access to services and promote self-sufficiency and empowerment.

· Program Compliance: Ensure services are delivered in alignment with agency policies, funding requirements, and applicable regulations.

· Family Engagement: Foster strong, supportive relationships with families to encourage ongoing participation and engagement in services and child development activities.

· Team Collaboration: Work closely with multidisciplinary teams (educators, health professionals, supervisors) to support coordinated service delivery.

 

The Perinatal Outreach Nurse reports to their Nurse Supervisor for weekly supervision and professional development. The PON is accountable for compliance with all applicable regulatory standards, policies, procedures, and protocols of the Human Resources Department, the Greater Dayton Area Hospital Association (GDAHA) and any funding sources that support the work of the department. This individual must maintain a Registered Nurse licensure through the Ohio Board of Nursing

1. Deliver structured, evidence-based curriculum during recurring home visits to promote school readiness, parent-child interaction, healthy development, and overall family wellbeing.

2. Adapt lesson plans and materials based on the family’s goals, cultural background, and child’s developmental level while maintaining fidelity to all applicable program requirements and guidelines.

3. Establish and maintain supportive relationships with families to build trust and encourage active participation in the program.

4. Assess family strengths and needs through interviews, screenings, and collaborative goal setting.

5. Develop and monitor service plans that address goals related to education, employment, health, housing, and child development.

6. Provide referrals and coordinate access to community resources such as healthcare, mental health, food assistance, housing, and employment services.

7. Document all contacts, assessments, referrals, and follow-ups in an accurate and timely manner, in accordance with program policies and confidentiality standards.

8. Collaborate with internal teams and external stakeholders to support coordinated care.

9. Support families through transitions, including entry to preschool, childcare, employment, or other programs.

10. Promote school readiness by supporting parent-child interactions and early learning activities during home visits.

11. Maintain compliance with all program standards, policies, and procedures, including mandated reporting laws and documentation requirements.

12. Participate in team meetings, supervision, and professional development to enhance service quality and stay informed of best practices.

13. Engage families in group activities or workshops when applicable, including parenting classes, health education, and family engagement events

14. Collaborate with leadership to identify learning needs and formulate self -development goals/developmental plan.

15. Assist with mentoring of home visitors/service coordinators/perinatal outreach nurses

Education: 

Minimum Level of Education Required: Bachelor's degree 

Additional requirements:

Type of degree: Bachelor's degree, will consider ADN with signed Memorandum of Understanding (MOU) to obtain BSN within 5 years of hire.

Area of study or major: Nursing

Preferred educational qualifications:

Position specific testing requirement: Must undergo a criminal background check with the Ohio bureau of criminal identification and investigation and a background check with the Ohio central registry on child abuse and neglect. 

Licensure/Certification/Registration: 

Licensed Registered Nurse 

Experience: 

Minimum Level of Experience Required: 1 - 3 years of job-related experience

Prior job title or occupational experience: Prefer experience in a variety of settings such as home visiting, OB/Gyn, Pediatrics, Adolescents, or Mental Health.

Preferred experience: At least 1 year working with pregnant women, infants, and children.

Other experience requirements: Complete orientation per the Nurse Family Partnership and Help Me Grow guidelines. 

Knowledge/Skills: 

Must possess and display effective verbal, written and interpersonal communication skills.

Organized and self-directed.

Valid driver’s license and insured safe automobile.
