Physical Therapist/ Home Care Case Manager

 

 

Position Summary

 

General Summary/Responsibilities:

The Physical Therapist evaluates and then directly provides or supervises the provision of physical therapy services to patients in their homes or residences.  When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient’s home care services.  The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.  The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.  In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

 

 

Qualifications

 

Education

 

Minimum Level of Education Required: Bachelor's degree

                                               

Additional requirements:

    • Type of degree: Bachelors - Graduate of an accredited Physical Therapy Program.
    • Area of study or major: Physical Therapy

    • Position specific testing requirement: N/A

 

Licensure/Certification/Registration

 

  • Current licensure to practice Physical Therapy in Ohio.  
  • CPR Certification

 

Experience

 

Minimum Level of Experience Required: 1 - 3  years of job-related experience

Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care.

Prior specific functional responsibilities: N/A

Preferred experience: One year clinical experience in acute care setting preferred  

Other experience requirements: N/A

 

Knowledge/Skills

 

 

  • Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.
  • Must be able to creatively adapt multiple environments and situations for successful outcomes.
  • Must be self-directed and able to work with little supervision.
  • Computer skills preferred.
  • Home care experience preferred.

 

Organizational Standards

 

 

  1. Demonstrates ability to follow clinical process:
  • Provides thorough physical, mental and social assessments of patients upon admission to home care agency as well as during each home care visit.
  • Establishes plan of care in coordination with patient, caregiver, family members, physician, case manager and other home care team members.  Revises plan of care as necessary.
  • Implements and delivers services according to the plan of care.  Assesses and documents the patient’s response to treatment.
  • Observes and monitors the medical status and rehabilitation progress of the patient on an ongoing basis with appropriate intervention.
  1. Complies with all agency regulations/policies/procedures:
  • Submits timely documentation as per policy.
  • Submits completed initial admission documentation as per policy.
  • Completes and submits patient recertification as per policy.
  • Follows agency and governing bodies’ standards of practice.
  • Responds to referrals in a timely manner as per policy.
  • Performs documentation using systems designated by Fidelity Health Care.
  1. Demonstrates effective communication skills when dealing with internal and external customers.
  2. Demonstrates effective case management skills:
  • When case manager, coordinates the delivery of services of all members of the home care team as designated by the plan of care.
  • Communicates with the case manager and/or other disciplines involved a minimum of every 62 days and/or at recertification or more often if condition warrants.
  • Communicates all pertinent clinical and psychosocial information to home care team members via daily visit notes, interdisciplinary reports, and case communications and conferences.
  • Demonstrates knowledge and utilization of home care rehabilitation and community resources.
  • Provides patient/family teaching regarding disease process and prevention, infection control, home safety, home exercise program, and management of the patient in the home setting related to rehab diagnoses.
  • Directs and supervises services delegated to and provided by the Physical Therapy Assistant, monitors frequency of visits and ensures compliance of the care plan.
  • Utilizes the Home Care Aide as an extension of therapy in cases where appropriate.
  • Notifies all disciplines, physician and home care agency of discharge and completes the discharge summary as per policy.
  1. Participates in agency quality assurance/improvement activities and team/work groups.
  2. Attends all scheduled inservices and staff meetings as per policy.
  3. Strives to maintain/improve home care and clinical education for self and/or community.
  4. Performs case workload review as designated by the supervisor.
  5. Appropriately arranges time off coverage and communicates this with the agency.
  6. Maintains average productivity expectation as established by the agency.  Is responsible for maintaining minimum weekend on-call, holiday or other requirements established by the department.
