The Substance Use Navigator is responsible for providing coordination and support to Premier Health Emergency Department /OB Triage patients and patients admitted to the medical floors who have been identified with substance use disorder (SUD). SUN will be responsible for supporting linkage and warm handoff for Medications for Addiction Treatment (MAT) from the ED and the inpatient medical floors. This includes, but is not limited to regularly performing SUD assessments, linkages to ongoing treatment, linkages to community resources, educating patients, communicating with, Clinical and administrative staff, monitoring and reporting outcomes, and participation in other grant-related meetings and activities. Additionally, ensures follow up regularly occurs with the identified patient population.
Essential Duties & Functions:
o Participate in the screening process that identifies patients with substance use disorder (SUD)/Opioid Use Disorder (OUD).
o Establish positive relationships with patients, treatment entities, community resources and peer recovery support resources.
o Coordinate a course of action to effectively follow up with patients who present in the Emergency Department ,OB Triage, and on the medical floor with SUD/OUD.
o Provide advocacy and act as a liaison between patient and emergency and inpatient medical department clinical staff.
o Assist patient with eliminating barriers to accessing MAT in the ED /OB Triage/Inpatient Medical Floors and ongoing treatment.
o Coordinate warm handoff to treatment within the ED/Inpatient Medical Floor to appropriate treatment entities (outpatient, inpatient, detox, or residential treatment)
o Develop documentation system to assure accurate follow-up, record keeping and program reporting
o Enter encounter data into electronic medical record system, EPIC, and other data collection systems
o Participate in regular meetings, education, and training pertinent to successfully conduct functions of the role.
o Other administrative duties as assigned.
Minimum Level of Education Required: Associate degree
Additional requirements:
- Type of degree: Associate
- Area of study or major: Preferred area - Allied Health, Chemical Dependency Concentration, or health services related field
- Preferred educational qualifications: Associate Degree in either Chemical Dependency, Allied Health, Social Services, or Social Work
- Position specific testing requirement: N/A
Licensure/Certification/Registration
- Chemical Dependency Counselor Assistant (CDCA) or will obtain within the first 6 months of employment
Experience
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Community Health Worker, Peer Recovery Supporter, Case Manager or emergency department experience.
Preferred experience: Medical Terminology, emergency department, substance abuse case management, grants
Other experience requirements: Experience with data/MS Office
