Premier Outpatient Behavioral Health - Montgomery County

FT / 80 hours per pay

Summary

The Therapist functions as a member of a team to assist in the planning, development, and delivery of outcome-effective treatment services in various types of settings. The therapist assesses the patient’s social, emotional, cultural, mental health, and family situation and interprets this information to develop a treatment plan to work with the patient on achieving established goals and objectives.

The therapist is skilled in individual, family, and couple’s therapeutic approaches that can be utilized to assist the patient in achieving the established goals. The therapist is able to work independently and collaborate with office physicians to provide the best continuity of care for the patient.

This position also functions to plan and deliver both on-site and off-site clinical case coordination, intervention, consultation and/or educational services to children, adolescents, adults, and families as needed.

Nature and Scope

The Therapist will demonstrate strong ethics, honesty, and adherence to Premier Outpatient Behavioral Health’s Code of Conduct, Compliance Program, Patient Experience philosophy, guidelines and tools.  The Therapist role will include duties such as:

  •   Provide individual counseling to patients as well as family intervention as needed and is also able to conduct family and couple’s therapy.
  •        Schedule, provide and document all clinical services accurately and according to departmental standards utilizing and documenting evidenced based practices.
  •        Collaborate with referral sources, internal team members and external educational and service providers to ensure appropriate patient care and continuity of care.
  •        Demonstrate clinical knowledge and skills relevant to the appropriate clinical services and intervention methods provided to patients and their families.
  •        Seek medical provider consultations regarding clinical needs of patients. Maintain on-going communication with medical providers for the purpose of coordinating and evaluating patient care.
  •        Demonstrate strong customer satisfaction skills in the provision of clinical care services and in coordinating care with referral sources and other stakeholders.
  •        Demonstrate knowledge of Premier Outpatient Behavioral Health clinical records and assigned Program and other paperwork standards. Apply competencies to both the provision and documentation of clinical records including resolving failed activities and alerts within required timeframes.
  •        Demonstrate compliance with agency standards for administration and reporting of outcome measures and documentation of utilization management of clinical care.
  •        Demonstrate knowledge of other regulatory standards. Ensure that clinical records are maintained in compliance with these standards at all times.
  •        Demonstrate knowledge of Information Technology policies and procedures. Apply them to the documentation of clinical care according to clinical records and other agency standards.
  •        Attend required staff meetings and training courses including annual Healthstream assignments. Participate in case conferences as needed.
  •        Meet consistently with Program Supervisor and as needed to review clinical care of patients according to licensing and agency standards and agency supervision plan.
  •        Ensure productivity standards are met consistently as applicable to departmental standards.
  •        Maintains licensure and attends appropriate CEU training.

Qualifications

·       Bachelor’s or Master’s degree in a relevant social science field

·       Licensed in the state of Ohio as a Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Licensed Social Worker (LSW), or Licensed Independent Social Worker (LISW) or Independently Licensed Marriage and Family Therapist (IMFT)

·       Training and experience in the types of services provided by the team for the adult population

·       Diagnostic and Statistical manual of Mental Disorders (DSM- 5 or subsequent revisions)
