Position: Therapist LSW/PC-Crisis-Preble

Dept: Preble County

Shift: Full Time Monday-Friday 8:00 am -5:00 pm /80 Hours per Pay

Facility: Samaritan Behavioral Health

This Therapist will be responsible for providing in-person, telehealth, and on-call Crisis support for Adult populations in Preble County.  SBHI is the mobile adult crisis responder in Preble County during our hours of operation.  This therapist shall provide for this service including but not limited to de-escalation, assessment, clinical support, and hospitalization (pink-slipping) clients who present as walk-ins or in the community.  This service is provided on-call during our hours of operation.  In addition to these responsibilities, this therapist shall monitor Crisis Notifications, emailed notifications of adults presenting to local EDs with codes consistent with mental health crises, and shall provide outreach to these patients with the goals of connecting them with ongoing support and services.  This Therapist may also be called upon to accompany Case Managers into the field to assist with the assessment and support of clients with SPMI.  This Therapist will also participate in and at times lead trainings for various stakeholders in the community around the topic of Crisis Intervention and De-escalation.

1.      Work collaboratively with the local Crisis Hotline, law enforcement, juvenile court, schools or other community agencies to receive calls for MRSS

2.      Outreach to the above indicated entities to publicize program (minimal contacts will be established)

3.      Triage referrals by calling the individual/family in crisis

4.      Face-to-Face Mobile Response time of 20-60 minutes.  These mobile locations may be at home, school, court, etc.  This timeframe can be accomplished through positioning staff in satellite locations. 

5.      Anticipated Face-to-Face response (vs. via phone only) will be at least 75% of all referrals/requests.

6.      Provide caregiving support up to 45-60 days

7.      Data collection/reporting on:

a.      Number of calls/referrals by Day & time

b.      Response time:  via phone and/or face-to-face

c.      Location of response

d.      Outcome of response

e.      Linkages achieved during Stabilization (how many and with what agency)

f.        Length of time involved in Stabilization services

g.      Amount billed/collected from Medicaid and Private Insurance

h.      Evaluation consent form completion by parent/guardian

i.        Note: Further components may be added or changed as the project develops

8.      Working with electronic medical records system and documenting patient interactions

9.      For sustainability purposes, services, to the fullest extent possible, should be billed to Medicaid and/or private insurance.

10.   Other duties as assigned

 

 

 Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), Licensed Marriage and Family Therapist (MFT)Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage & Family Therapist (LMFT)

§  Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), Licensed Marriage and Family Therapist (MFT)Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage & Family Therapist (LMFT)

§  Certified in CPR within 30 days of hire

§  Valid Driver’s License and personal vehicle required

 

Minimum Degree:  Bachelor's Level, Master's Preferred

 

 

 Experience working with at-risk and vulnerable individual youth/family systems
