§  Health Coach certificate preferred

§  Excellent interpersonal skills; ability to empathize; service-oriented attitude

§  Ability to work well with people at all levels

§  Excellent communication and organizational skills

§  Skilled in gathering and reporting information, as well as record keeping

§  Demonstrated problem solving and decision-making skills with ability to exercise initiative

§  Ability to maintain confidential information

§  Pass state and federal background checks

§  Computer skills necessary

§  Must be capable of standing, stooping, bending and lifting up to 50 lbs.

Working Conditions:       

 

1.       General office environment.

2.       Climate controlled environment.

3.       May work some evenings and weekend hours to complete projects or meet the needs of the department.

4.       May be required to work overtime as defined by the needs of the department.

 

Organizational Standards

§  Works with patients with readmission issues to prevent reoccurrence

§  Provides support, empowerment education and case management services, as designated

§  Assist patients in meeting their goals, according to the plan of care, and improving self-management skills

§  Educates patients about the health care system and facilitates relationship building between the two

§  Assist patients in their chronic disease management as directed by the nurse Navigator

§  Schedule and accompany patients to their appointments, as necessary

§  Coach patients to schedule their own health care appointments and transportation

§  Coach patients to better their communication with physicians, nurses, and other members of the health care system

§  Identifies support needs for patients and their families

§  Makes certain patients have access to prescriptions, DME equipment, physician and outpatient services as needed

§  Assist patients in addressing psychosocial needs and self -management as directed by the social worker

§  Assist patients to arrange for food and housing

§  Assists with phone calls and follow up for patients in different levels of the program

§  Document interventions in patient electronic record

§  Communicates orally and in writing with patients, physicians and staff as indicated

§  Participates in performance improvement activities as assigned

§  Serve as an integral part of the Advance Illness Management Team

 

Essential Duties & Functions

Essential Duties & Functions:

Must have available phone to communicate and transportation, with appropriate licensure and insurance, to visit homes and other sites.  Interacts with physicians, nurses, social workers and other disciplines, administrative personnel, and community resources.
