Position information               

TitleCommunity Outreach Liaison

Facility: Fidelity Health Care

Reports To: BCCP Program Manager                

 

General Summary/Responsibilities:

The Community Outreach Liaison will develop positive working relationships with community partners, providers, and team members. The Community Outreach Liaison will track via team calendar and accept/organize events based on workplan goals. If event is unable to be attended, the Community Outreach Liaison will ensure material is provided to the organization for event. Additionally, the Community Outreach Liaison will travel to attend community events and go to provider sites to initiate/continue educational and programmatic outreaches utilizing the workplan and targeting communities with significant health disparities. The Community Outreach Liaison will assist team members with any additional task needed to help accomplish workplan goals and success of grant.
